Four Seasons Under-Eye Fat KEY POINT

  1. KEY POINT 01

    Providing the optimal solution

Because the cause and appearance of protruding under-eye fat differ from person to person, we precisely analyze why it occurs in each individual and provide the optimal solution.

  2. KEY POINT 02

Triple combination treatment (fat removal + fat repositioning + septum reinforcement)

We remove unnecessary fat and reposition the remaining fat into hollowed areas, then perform septum reinforcement to prevent the fat from protruding again, delivering the best possible result.

  3. KEY POINT 03

    Aegyo Muscle Preservation

Under-eye fat is entirely different from aegyo-sal. For patients who have an existing aegyo muscle, surgery is performed without damaging the aegyo-sal. For those without aegyo-sal, we create it to form a natural contour line.

  4. KEY POINT 04

    Preventing Under-Eye Fat Recurrence

During under-eye fat repositioning, the septum between the skin and the fat is reinforced to create firm fixation. This prevents the side effect of fat bulging out again, as well as recurrence.

  5. KEY POINT 05

Surgery Using Only Your Own Fat, With No Foreign Materials (Fillers)

Rather than using fillers, a temporary measure, to fill only the hollow areas, we remove unnecessary fat and reposition the surplus fat into the hollowed areas for a natural result.

What Is Under-Eye Fat Repositioning?

After carefully assessing the degree of under-eye fat protrusion, hollowing, and skin condition, this surgery tidies up unnecessarily protruding fat and repositions it where it is needed.

Tailored to each person's under-eye structure, it improves uneven contours and shadows to create a natural, smooth under-eye line.

Causes of Under-Eye Fat

There are three fat pads beneath the eyeball. As the elasticity of the septum, muscle, and skin surrounding the fat decreases, the under-eye area bulges out and under-eye fat develops.

When under-eye fat protrudes, it creates shadows and wrinkles under the eyes, giving a tired and older-looking impression.

It does not occur in everyone, and its causes can be broadly divided into aging and congenital factors.

Under-Eye Fat Caused by Aging

  • When the skin around the eyes is very thin or lacks elasticity
  • When the cheeks sag

Under-Eye Fat Caused by Congenital Factors

  • When you are born with a large amount of under-eye fat
  • When the front part of the cheekbones is genetically underdeveloped
  • When the eyeballs protrude

How Under-Eye Fat Develops

  • Normal Under-Eye Fat

    • The under-eye fat is well positioned within the orbit, with no protruding fat.
    • The skin, muscle, and fat in front of the under-eye fat maintain good elasticity and support it.
  • Early Under-Eye Fat Protrusion

Cross-section illustration of an eye in which under-eye fat has begun to push forward
    • As the elasticity of the surrounding tissue declines, the under-eye fat begins to protrude.
    • The surrounding tissues, having lost their elasticity, can no longer support the under-eye fat and begin to sag downward.
  • Advanced Under-Eye Fat Protrusion

Cross-section illustration of an eye with significantly protruding under-eye fat and a tear trough beneath it
    • As the under-eye fat continues to protrude, skin sagging also develops.
    • As the surrounding tissues sag together, a hollow tear trough forms beneath the protruding under-eye fat.

Under-Eye Fat and Aegyo-sal

Under-Eye Fat and Aegyo-sal

Under-eye fat and aegyo-sal are anatomically different structures, in different locations, with different shapes.

Aegyo-sal is formed by the muscle surrounding the eye gently rising, making the face look bright and defined, as well as lively and youthful.

When under-eye fat develops, the aegyo-sal can be buried by the fat and disappear, or fold double and look like wrinkles.

At Four Seasons Plastic Surgery, under-eye surgery is performed while preserving your original aegyo-sal rather than removing it, creating a lively, natural facial line.

Under-Eye Fat Surgery by Type

Depending on the type of under-eye fat, we divide treatment into 8 main surgical methods. After a detailed and accurate consultation with our medical team, surgery is performed based on a diagnosis suited to each person's eye shape.

FOUR SEASONS LOWER EYELID FAT

Surgical Methods and Suitable Candidates

  • 01

    One-Day Under-Eye Fat Repositioning

Improve your under-eye fat in just a single visit. Because this method uses dissolvable sutures for internal fixation from inside the skin, no suture removal is needed and you can return to daily life right after surgery.

    Suitable Candidates

    • 01Those in their 20s to early 40s whose skin is not too thin and who have little to no sagging
    • 02When under-eye fat becomes more noticeable as you smile
    • 03When only the inner part of the under-eye fat protrudes
    • 04When you want to return to daily life immediately after surgery
  • 02

    Soft Under-Eye Fat Repositioning

For patients with thin skin, anchoring the suture on the inner side of the skin can cause dimpling at the fixation point. In these cases, we can perform soft under-eye fat repositioning, in which the suture is brought out to the outer side and secured there.

    Suitable Candidates

    • 01When thin skin makes one-day under-eye fat repositioning surgery difficult
    • 02When the area around the eyes looks hollow and sunken
    • 03When only the inner part of the under-eye fat protrudes
    • 04When you have time to spare before returning to daily life after surgery
  • 03

    Dual Under-Eye Fat Grafting

When the amount of under-eye fat is small, or repositioning alone would have only a minimal effect, under-eye fat grafting is performed together with repositioning to improve under-eye hollowing.

    Suitable Candidates

    • 01When the under-eyes are deeply sunken
    • 02When lost elasticity makes the under-eye groove prominent and casts a shadow
    • 03When dark under-eyes leave you concerned about dark circles
    • 04When repositioning alone would have only a minimal effect
  • 04

    Eye Shurink Repositioning

This procedure is for those with mild under-eye fat protrusion and fine lines who may have some skin laxity after under-eye fat surgery. One month after the under-eye surgery, a Shurink lifting laser is used to restore firmness to the sagging skin.

    Suitable Candidates

    • 01When there are slight fine lines under the eyes
    • 02When skin laxity is possible after under-eye fat repositioning surgery
    • 03When you are in your 20s to early 40s with almost no under-eye skin laxity
    • 04When under-eye fat becomes noticeable when you smile
  • 05

    Hybrid Lower Blepharoplasty

This procedure is suited to patients in their 40s to 60s without severe skin laxity. Under-eye fat is removed and repositioned through a transconjunctival incision. A minimal incision is then made along the lower lash line, and the loose skin is pulled up and secured.

    Suitable Candidates

    • 01When under-eye fat protrudes and the skin sags along with it
    • 02When under-eye fat becomes more pronounced when you smile
    • 03When under-eye fat protrudes on both the inner and outer sides
    • 04When severe skin laxity makes you look older than your age
  • 06

    All-in-One Lower Blepharoplasty

This method is suited to those whose under-eye fat and skin have sagged together because of age-related loss of skin elasticity, and who need correction through skin incision and excision beneath the lashes. It can improve under-eye wrinkles and the shadowed look they cause.

    Suitable Candidates

    • 01When under-eye fat protrudes and the skin sags along with it
    • 02When under-eye fat protrusion and skin laxity are severe
    • 03When under-eye fat casts a shadow beneath the eyes
    • 04When severe skin laxity makes you look older than your age
  • 07

    Triple-Fixation Lower Blepharoplasty

This procedure is suited to patients with severely protruding under-eye fat and marked skin laxity that has caused the lower eyelid to turn outward. To keep the eversion from recurring, the fat and skin are lifted and secured more firmly with triple fixation.

    Suitable Candidates

    • 01When the lower eyelid skin has turned outward
    • 02When under-eye fat protrusion and skin laxity are severe
    • 03When there is a chance of recurrence soon after surgery
    • 04When severe skin laxity makes you look older than your age
  • 08

    Lower Blepharoplasty Lifting

This procedure is suited to those who are often told they look older because the front of the cheeks sags along with the under-eyes, making the nasolabial folds look deeper. While the under-eye fat surgery is performed, the sagging front cheek muscle and skin are lifted together, giving the face a more vibrant look.

    Suitable Candidates

    • 01When under-eye fat has developed in a flat face with sagging front cheeks
    • 02When you want to improve overall midface sagging
    • 03When severe skin laxity makes you look older than your age
    • 04When under-eye fat casts a shadow beneath the eyes
