Revision Eye Surgery

Removing all the
unnecessary tissue that causes problems,
then rebuilding with your own clean tissue —
that is true revision eye surgery

When Revision Eye Surgery Is Needed

Fixing the tissue too tightly without regard for anatomical structure, or removing too much skin, muscle, or fat, can lead to the need for revision double eyelid surgery.

Among revision eye procedures, sausage-eye revision and epicanthoplasty reversal are complex and difficult cases, so it is best to have them performed by a revision-specialist medical team with extensive experience.

  1. Eye illustration — the double eyelid line is set too high, making the upper eyelid puff up thickly like a sausage
    Case 01

    Line too high (sausage eye)

  2. Illustration of both eyes — both have double eyelids, but one line is higher and wider, making the two sides uneven
    Case 02

    Asymmetric line

  3. Eye illustration — the double eyelid line has loosened and thinned, fading or breaking in places
    Case 03

    Loosened & thin line

  4. Eye illustration — multiple crease lines layered over the double eyelid
    Case 04

    Layered crease correction

  5. Eye illustration — an under-corrected eye with a drooping eyelid covering the upper part of the dark pupil
    Case 05

    Under-correction / Over-correction

  6. Eye illustration — the inner corner of the eye is opened excessively toward the nose, exposing pink flesh
    Case 06

    Excessive epicanthoplasty

Revision Eye Surgery: Why It Is Difficult

Highly Complex Revision Eye Surgery

Epicanthoplasty Revision

Epicanthoplasty revision is not simply about returning the eyes to their state before surgery. Its purpose is to correct complications that developed after epicanthoplasty through a secondary procedure, giving the eyes a natural shape.

Therefore, it does not restore the original epicanthal (Mongolian) fold. Instead, it is a reconstructive procedure that reshapes the inner corner of the eye to suit the current condition of the eye.

    Shape Problems
    • When a lot of red tissue is visible at the inner corner of the eye
    • When the inner corner looks pointed or has a boxy, bracket-like shape
    • When the epicanthoplasty results are asymmetrical between the two eyes
  • Diagram of an epicanthoplasty scar problem with a red incision scar remaining at the inner corner of the eye
    Scar Problems

    • When a sunken or depressed scar remains

    • When the space between the eyes has narrowed, making the face look cramped
    • When the epicanthoplasty doesn't suit the patient's own image

Four Seasons' Signature Epicanthoplasty Revision Technique

Triangular Flap

Using a Triangular Flap

Scar tissue from a previous epicanthoplasty can leaveareas with a shortage of skin. By filling in the deficient area with a triangular flap,a natural epicanthoplasty revision result can be achieved.

Advantages of Epicanthoplasty Revision

  • Eye diagram showing the inner corner closed with fine, close sutures to reduce scarring

Restores even the ligament and muscle of the medial canthus to minimize scarring

  • Eye diagram showing the skin deficiency at the inner corner filled with a triangular flap

Uses a triangular flap to fill areas where skin was lost in the previous surgery

  • Eye diagram showing the eye-opening width measured and designed section by section

Designed down to a 1 mm difference to suit your overall image

Highly Complex Revision Eye Surgery

Correcting Sausage-Eye Double Eyelids

Even when the double eyelid thickness is the same, a sausage-like double eyelid may or may not form depending on each person's eye shape and skin condition. In most cases, sausage eyes result from setting the line too high, creating a bulky fold.

  • 01
Eye diagram showing a bulky double eyelid line caused by fat left in the upper eyelid
When the upper eyelid fat was not removed
  • 02
Eye diagram showing a high line set on thick skin
When the line height was decided without considering skin thickness
  • 03
Eye diagram showing the double eyelid fixation angle pointing upward
When the double eyelid fixation angle points too far upward
  • 04

When the double eyelid is too large for the eye-opening strength

Revision Methods for Sausage Eyes

  1. Eyelid diagram showing a new incision line that includes the existing incision line — when there is excess skin

When there is excess skin: Double eyelid revision

    1. 01.Incision made to include the existing incision line
    2. 02.Skin excised depending on the condition of the eye
    3. 03.The previously high-set double eyelid line is released, and the newly created line is fixed at a lower position
  2. Eyelid diagram showing a new incision line as a dotted line at a lower position below the existing line — when there is no excess skin

When there is no excess skin: Two-line technique

    1. 01.Skin from the existing line is not removed
    2. 02.A new incision is made at a lower position to create the line
    3. 03.The existing double eyelid line is dissected and released, and fat repositioning is performed to prevent re-adhesion
